Joining Is Simple
Membership plan terms
Yorkville Family Dental membership plans provide discounted fees on eligible dental treatments performed at our office during the active membership period. This plan is not dental insurance, cannot be combined with insurance or other promotions, and does not include whitening services, specialty referrals, or services not listed in the plan. Full payment is due at enrollment, membership fees are non-refundable, and Yorkville Family Dental does not submit insurance claims for services provided under the plan. Members are responsible for payment of all services rendered, and missed or canceled appointments without proper notice may result in forfeited services.
